  1. Fill out a DS-11 application in Black Ink Only (you can print out the application at the U.S. Department of State - Bureau of Consular Affairs website) before you come in for your appointment, do not sign until in front of an agent
  2. Bring a Passport Photo
  3. Bring the current Passport
  4. Bring a copy of your Birth Certificate
  5. Bring a copy, front and back, of valid (must be Driver's License, State ID Card, current Passport or Military ID and cannot have the statement "Not For Federal Purposes" on the top of the ID)
