In order to apply for a passport at the City of Laurel Passport Office, you MUST have everything with you, including a filled out application, BEFORE seeing the passport agent OR YOU MAY BE ASKED TO COME BACK ANOTHER DAY.


The City of Laurel is an authorized passport acceptance agent.   Please note, however, that the City of Laurel is not authorized to process official, military or "no fee" passport applications.

NEW REGULATIONS



1)  Beginning April 1, 2011, the U.S. Department of State will require the full names of the applicant’s parent(s) to be listed on all certified birth certificates to be considered as primary evidence of U.S. citizenship for all passport applicants, regardless of age.  Certified birth certificates missing this information will not be acceptable as evidence of citizenship.  This will not affect applications already in-process that have been submitted or accepted before the effective date.

In addition to this requirement, certified copies of birth certificates must also include the following information to be considered acceptable primary evidence of U.S. citizenship:



  • Full name of the applicant

  • Date of birth

  • Place of birth

  • Raised, embossed, impressed or multicolored seal of issuing authority

  • Registrar’s signature

  • The date the certificate was filed with the registrar’s office (must be within one year)

2)  As of October 30, 2010, the Department of State will ONLY accept Puerto Rican birth certificates on or after July 1, 2010. Affected customers that do not yet have a new Puerto Rican birth certificate should visit

This new law WILL NOT affect Puerto Rican citizens who currently have a US Passort and are eligible to submit Form DS-82: Application for a US Passport by Mail.

The City now accepts Visa or Master Card for payment of the Processing Fee. 

Passport fee must be by check or money order payable to "US DEPARTMENT OF STATE".



  • $60 Expedite fee for each application (2 - 3 week return).  Additional charges will apply to optional overnight delivery service.  To receive your passport as soon as possible, we strongly suggest that you arrange overnight delivery service for:

    * Sending your passport application
    * Returning your passport to you.
     

  • Fees must be paid at the time of application. Other fees may be applicable as well.

    Passport fees paid at the time of application are non-refundable processing fees.

 


What documentation is required?

(If you do not have all of the below, you may be asked to return another day, so please be sure to carefully read and bring all needed documentation with you)
 


Application
 



  1. A DS-11 Application is needed for new passports or renewals, so please be sure to have a completed application when you appear before the passport agent.

  
Proof of U.S. Citizenship
 



  1. A previous U.S. Passport
     

  2. If you were born in the U.S., the original or a certified copy of your birth certificate issued by the State, County or City of your birth that contains a registrar's raised, embossed, impressed or multi-colored seal and the date the certificate was filed with the registrar's office.
     

  3. If you have neither a certified birth certificate or a passport bring a notice from the registrar of the State where you were born that no birth record exists.  Also, bring as many as possible of the following:  a baptismal certificate, hospital birth record, early census, school record or family bible record (to be considered, these items must show your full name and date and place of birth).
     

  4. If you were born abroad, bring a certificate of naturalization, or a certificate of birth abroad.
     

  5. Applicants 17 years old and under must appear with a parent or legal guardian.  Applications for children 15 years old and under must meet the "2-parent consent" guidelines and the consent form must be notarized, as well as the form must be accompanied by a copy of the absent parent's Driver's License, front and back.  When renewing passports for children under age 14, both the original birth certificate and the previous passport must be submitted.  See Children's passport issuance link below.

  1. A previous U.S. Passport

  3. A valid driver's license, government or military ID or other photo ID (You must now provide a copy of your driver's license, front and back, that will be sent along with your application. If you do not have one when you appear to apply for your passport, the City of Laurel can make a copy for you, but applicant will be charged $1 per copy, per application for said service)

 


One Photograph

One (1)  full front pose, 2" by 2" color photograph on white or light-colored background. (photo must be recent, taken within the last six months).  No vending machine photo will be accepted.


 



Childrens Passport Issuance Alert Program
 


Separate from the two-parent signature requirement for U.S. passport issuance, parents may also request that their children's names be entered in the U.S. Passport name-check system. The Children's Passport Issuance Alert Program provides:
 



  1. Notification to parents of passport applications made on behalf of their minor children
     

  2. Denial of passport issuance if appropriate court orders are on file with the CPIAP.
